Use the supply order form options below for convenient reordering of your Redwood Toxicology Laboratory, Inc. laboratory supplies and/or rapid
point-of-care drug test devices.

REQUIREMENTS FOR REORDERING SUPPLIES

This form is only available to existing clients with an account number. Complete the form with the company information that corresponds to your
account and contract.

Please allow 5-7 business days to receive orders.

Overnight or rush delivery charges will be billed to your account.

For lab supplies, unless otherwise specified, we will send a three-month supply.

The type of supplies sent to your agency and pricing for each supply or shipment will depend upon individual contract agreements. You will be contacted by a sales representative if you select supplies that are not contractually agreed upon or request a quantity of lab supplies that significantly exceed your normal
3-month volumes.
